Following the introduction of the new European GDPR policy in May 2018, all patients who wish to have personal documentation collected from the surgery by another person including;

Prescriptions
Test Results
Sick Certificates
Work Notes
Invoices/Bills
Referral letters
Dept of social security forms/correspondence
Medical Legal forms/correspondence

Will need to sign and return this form correspondence consent2 and return it to the surgery.
If someone is bringing the form to us on your behalf, they will be required to provide a copy of your either your passport, drivers licence or public services card to verify your signature.
